Medical Social Worker Hospice Part-Time/PRN

Medi Home Hospice, a proud member of the Medical Services of America family, is currently seeking an experienced Part-Time/PRN Medical Social Worker for our Hospice Patients in Greensburg and Westmoreland, PA.· Provides social work services in accordance with the physician’s plan of treatment.· Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.· Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.· Serves as a liaison with other community resources.
