We are looking for a dedicated Nurse Practitioner who shares that passion to join our team. Working alongside our Medical Director and collaborative care team, you’ll provide primary care to patients of all ages—managing acute and chronic conditions, delivering preventive care, and helping us build a healthier community. What You’ll Do:
Provide whole-person primary care, from routine check-ups to chronic disease management (diabetes, hypertension, asthma, behavioral health). Order tests, diagnose, prescribe, and create clear treatment plans tailored to each patient. Partner closely with our nurses, physicians, and care coordinators to deliver seamless, patient-centered care. Keep thorough, timely records in our EHR and support our quality improvement and HRSA compliance efforts. Step out into the community periodically for health screenings, education, and outreach events.
What You’ll Need:
License & Certs: Active Virginia Nurse Practitioner license, national FNP certification, current DEA registration, and BLS/ACLS.
Education: Master’s or Doctorate in Nursing from an accredited program.
Experience:
1–3 years in primary care or community health preferred (new grads with a true heart for FQHC work are welcome to apply!).
Bonus Points: Familiarity with FQHC operations, UDS measures, or quality improvement.
