American Traveler is hiring an experienced RN Case Manager for an acute care Care Coordinator role requiring EPIC experience and at least 12 months of acute care case management background. Details Hospital-based Care Coordinator/Case Management unit Day shift schedule, Monday-Friday 8:00am-4:30pm with no weekend rotation Caseload of approximately 16-25 patients per day Uses EPIC EMR system13-week contract duration
Requirements:
- Active and clear NY RN license required
- Current BLS certification requiredPRI and Screen certification preferred
- Minimum 12 months of acute care nursing experience required
- Minimum 12 months of acute care case management experience in a hospital setting requiredEPIC EMR experience required
Additional Information:
- Role focuses on acute care case management, discharge planning, and utilization management
- This role operates differently from other case management positions, making relevant experience critical to success
- Travelers receive 3 days of training and are expected to function at full capacity afterward
