Licensed Master Social Worker

Location: New York, NY 10016

Contract Duration: 13 weeks + probable extension

Shift: Day Shift (9:00 AM – 5:00 PM, Monday–Friday)

Pay Rate: $61/hr - $64/hrRequired Qualifications

Responsibilities: Attend interdisciplinary team meetings and unit rounds. Conduct patient screenings, psychosocial assessments, and care planning. Develop comprehensive and safe discharge and aftercare plans in collaboration with the interdisciplinary team. Coordinate discharge planning, including referrals to home care agencies and skilled nursing facilities (SNFs).Meet with assigned patients at least once per week, unless clinically inadvisable. Complete assessments, progress notes, care plans, and patient education documentation within required time frames. Utilize Allscripts, CarePort, and Parachute applications for referrals and orders. Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care.
