Job Summary: A well-established and highly regarded post-acute care organization near Rome, NY is seeking an experienced MDS Coordinator (RN) to join its clinical leadership team. This is a confidential search for a key role responsible for ensuring the accuracy, compliance, and integrity of resident assessments and care planning. Must Have: MDS 3.0 Experience
Requirements: Valid New York RN License Long Term Care Experience Required! Must be highly organized, professional & motivated Should have solid computer skills Excellent communication skills Should be friendly and a team worker
Key Responsibilities: Complete accurate MDS assessments and comprehensive care plans Initiate and oversee care planning processes and related activities Develop and distribute monthly care plan schedules in a timely manner Maintain and update all clinical assessments in accordance with regulations Conduct ongoing audits of clinical documentation for accuracy and compliance Partner with nursing leadership to address and resolve documentation issues Ensure strict confidentiality of all resident and organizational information#LI-DNI
