Description:
Conduct evaluations of patients in inpatient and outpatient environments, crafting tailored treatment strategies
Provide physical therapy interventions that target mobility, strength, balance, and functional capabilities
Work in collaboration with nurses, physicians, case managers, and a multidisciplinary rehab team to enhance patient outcomes
Guide patients and their families through therapeutic objectives, discharge planning, and at-home exercise programs
Maintain meticulous records of evaluations, treatments, and progress in alignment with facility protocols and regulatory standards
Engage in care team meetings and interdisciplinary conferences to guarantee coordinated patient care
Education Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications:
State licensure as a Physical Therapist
CPR certification is preferable
Additional specialized certifications (e.g., neurology, geriatrics) are advantageous but not mandatory
Skills Prior experience is strongly recommended
Ability to manage patients with intricate medical and mobility challenges
Excellent communication and collaborative skills
Familiarity with digital charting and clinical documentation systems
