RN MDS

Essential Duties And Responsibilities: Conducts required initial, quarterly, annual, and significant change assessments based on Resident Assessment Instruction (RAI) guidelines to ensure compliance with state and federal standards. Collects, analyzes, and documents data about each resident's health condition and medical history. Develops, revises, and reviews person-centered care plans as necessary. Communicates frequently with residents and their representatives and coordinates care plan meetings. Collaborates and effectively communicates with nursing staff and facility personnel to coordinate quality care for residents. Attends Staff/Unit meetings promoting, developing, and fostering a positive working environment. Functions as an integral part of the Quality Improvement Program and contributes to improving the facility's overall quality of care provided to residents. All other duties as assigned.

Supervisory Responsibilities: This position does not have supervisory responsibilities.

RN License: Must have prior experience with MDS assessments and care plans. Certified as a Resident Assessment Coordinator. Strong assessment and clinical skills. Proficient in electronic health record systems and MDS software. Effective communication and interpersonal skills.
