Description:
Assess patients in both inpatient and outpatient environments and tailor unique treatment plans based on individual needs
Administer therapeutic interventions aimed at improving mobility, strength, balance, and overall function
Work collaboratively with a diverse team of nurses, physicians, case managers, and rehabilitation professionals to enhance patient outcomes
Guide patients and their families on therapeutic goals, discharge strategies, and home exercise regimens
Ensure thorough documentation of evaluations, treatments, and patient progress in line with institutional and regulatory standards
Contribute to care team conferences and interdisciplinary meetings to foster coordinated patient care
Education Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications:
State licensure as a Physical Therapist
CPR certification preferred
Specializations (such as neurological or geriatric) are advantageous but not mandatory
Skills Prior experience is highly desirable
Aptitude for treating patients with intricate medical and mobility challenges
Effective communication and cooperative teamwork abilities
Familiarity with digital charting and clinical documentation software
