Patient Care Navigator — Primary Health Support

Central City Concern seeks a Health Assistant to join the Old Town Clinic team in Portland. You will support a multi-disciplinary primary care team serving homeless and low-income patients, ensuring timely communication, documentation and outreach tied to health services. The role requires a high school diploma or GED (Bachelor’s in health or social preferred), CPR certification, and ability to pass required screenings. Spanish fluency is a plus and union protections apply.
