Physician - MD/DO - Geriatrician

Position Overview: We are seeking aBoard-Certified Physicianto join our established geriatric medicine team serving skilled nursing facilities. This position offers complete control over your weekly schedule, predictable hours, and the opportunity to provide high-quality elder care.

Location: Monroe, WI (serving southwestern Wisconsin and northern Illinois communities)

Primary Duties: Provide comprehensive medical care to residents in skilled nursing facilities across the Monroe area. Conduct routine assessments, medication management, and care plan development Respond to acute changes in patient condition during your scheduled hours Supervise and collaborate with a team of experienced Geriatric Nurse Practitioners Complete documentation in the EPIC EMR system Participate in interdisciplinary care team meetings Communicate with patients, families, and facility staff regarding treatment plans Patient Population Established patient panel in long‑term care and post‑acute settings Geriatric patients with complex medical needs

Required Qualifications: Board Certification in Family Medicine OR Internal Medicine Dual Licensing: WI and IL Active medical license in Illinois (or ability to obtain prior to start date) DEA certification Current BLS certification Strongly Preferred Previous experience in post‑acute care settings Previous experience in long‑term care or skilled nursing facilities Geriatric medicine experience or specialized training

Salary Range: $135,000.00 to $857,000.00 / Year
