Geriatric Licensed Practical Nurse (LPN)

Bethesda Health is seeking a compassionate Licensed Practical Nurse to provide evening (7pm) care for seniors in our faith-based, nonprofit community. The LPN will deliver skilled nursing care, administer medications, monitor resident conditions, and support rehabilitation and chronic disease management. Responsibilities include documentation, coordination with RNs and interdisciplinary teams, and communication with residents and families. Provide direct bedside nursing care to senior residents on evening shift Administer medications and treatments per physician orders and facility protocols Monitor, assess, and document resident conditions and report changes promptly Support rehabilitation plans and chronic disease management Collaborate with RNs, therapists, and interdisciplinary team members Maintain accurate, timely electronic health records and care documentation Educate residents and families on care plans and health needs Uphold infection control, safety, and regulatory standards Promote person-centered, faith-informed, and dignified care Participate in shift handoffs, care conferences, and quality initiatives Medication administration Wound care management Vital signs monitoring Electronic medical records (EMR) documentation IV therapy (per scope and facility policy) Infection control practices Resident assessment and monitoring Care planning support Geriatric nursing care Patient and family communication Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN license in state of practice BLS/CPR certification Experience in long-term care, skilled nursing, or geriatric settings preferred Familiarity with electronic medical records (EMR) systems Knowledge of infection control and safety standards
