In this patient-first, mission-driven role, you will perform comprehensive PT evaluations, develop individualized treatment plans, and deliver therapeutic interventions for diverse adult and geriatric populations. Collaborate with physicians, nurses, and rehab teams, document care in the EMR, and support safe discharges. Perform comprehensive physical therapy evaluations and develop individualized treatment plans Deliver evidence-based therapeutic interventions, including exercise, gait training, and manual therapy Collaborate with physicians, nurses, and rehab professionals on coordinated care plans Document assessments, treatments, and progress in the electronic medical record accurately and timely Educate patients and families on home exercise programs, safety, and recovery expectations Participate in discharge planning to support safe transitions of care Adhere to regulatory, safety, and infection control standards Contribute to quality improvement and clinical best practice initiatives Flexibly cover PRN shifts across assigned hospital or outpatient locations Engage in ongoing professional development and training opportunities Physical therapy evaluation and treatment planning Orthopedic xevrcyc and neurologic rehabilitation Gait and balance training Therapeutic exercise prescription Manual therapy techniques Patient and family education Interdisciplinary care coordination EMR documentation Outcome measurement and reporting Infection control and safety protocols Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) preferred Basic Life Support (BLS) certification Experience with electronic medical records (EMR) systems
