Physical Therapist

What You’ll Do: Perform comprehensive physical therapy evaluations and interpret findings to determine the nature and extent of functional limitations. Develop individualized treatment plans and measurable therapeutic goals. Provide direct physical therapy treatment and modify treatment plans based on patient progress and clinical findings. Coordinate care with physicians, advanced practice providers, nursing, and other members of the healthcare team as appropriate. Educate patients and families regarding treatment plans, therapeutic exercises, mobility, injury prevention, and ongoing wellness. Assess patient progress toward established goals and determine when treatment modifications are necessary. Initiate discharge planning and appropriately discharge patients once maximum therapeutic benefit has been achieved. Maintain accurate and timely documentation of physical therapy services and patient management. What We’re Looking For: EducationGraduate of an accredited Physical Therapy program. Bachelor’s, Master’s, or Doctoral degree in Physical Therapy as applicable to professional training and licensure requirements.

Licensure: Current New York State Physical Therapist licensure/registration required.

Experience: Two or more years of Physical Therapist experience preferred. Outpatient, ambulatory care, community health, or multidisciplinary healthcare experience is highly desirable.
