Your Responsibilities Will Include: Carrying out thorough patient evaluations, ordering necessary diagnostic procedures, and creating individualized treatment plans Diagnosing and managing acute, episodic, or chronic conditions, either independently or in partnership with physicians, depending on your credentials Educating patients and their families to promote wellness, prevent illness, and empower them in disease management Prescribing medications and therapies in adherence to your practice scope Engaging in patient follow-ups and chronic care coordination Collaborating with multidisciplinary teams to ensure comprehensive, cohesive patient care Maintaining meticulous documentation within electronic medical records (EMR) Educational Background Needed Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Completion of an accredited Nurse Practitioner training program
