Rehab Clinical Navigator: Referrals & Patient Care

PAM Health Rehabilitation Hospital of Tulsa is seeking a Clinical Navigator. This role faces patients at their most challenging times and communicates rehab potential and support to help them reach their goals. Reports to the Director of Business Development and focuses on building relationships with referral sources. You will cultivate referral networks, manage the assigned territory, complete patient assessments, and perform sales contacts with referral and admission data analysis.
