Youth Transition Coach Independence & Growth

Southwest Network seeks a Transition Facilitator to coach teens and young adults through the Transition to Independence Process. You will support employment, education, housing, and wellbeing as they move from child to adult services, in community settings with flexible hours. You will work with partners across health, education, and welfare, developing individualized service plans and evaluating progress toward completion.
