Building trusted relationships with multilingual individuals and families, the full-time Multilingual Community Health Coordinator will provide culturally responsive outreach, education, and advocacy while navigating healthcare and social-service systems in a hybrid role that requires both remote work and onsite presence across Northern NY and VT.
Key responsibilities: Facilitate outreach, education, and resource connection to address barriers to care for multilingual communities Co-chair the Multilingual Patient and Family Advisory Council, ensuring multilingual perspectives inform service improvements Identify community needs and access barriers, sharing feedback to support community health assessments and improvement plans
Required qualifications: Associate degree or additional education in community health, public health, or a related field preferred Community Health Worker certification or completion of a recognized training program preferred Experience working with multilingual, immigrant, or culturally diverse communities required Fluency in English and at least one additional language commonly spoken by the communities served strongly preferred Relevant experience in community outreach, health education, or advocacy preferred
