Diabetes Educator (RN/RD) — Travel 2–3 days/wk

Advocate Health in Charlotte, NC seeks a full-time diabetes educator to lead DSMES sessions for patients with diabetes or related conditions. The role includes inpatient and outpatient education, with RN or RD licensure and possible CDCES certification. You will travel to several clinic locations 2–3 days weekly as part of patient care continuity. The educator will collaborate with physicians, nurses, and dietitians to tailor plans, monitor progress, and document services per CMS guidelines.
