Rehab and Long-Term Care Registered Nurse or Licensed Practical Nurse

The University of Vermont Health Network is seeking compassionate Registered Nurses and Licensed Practical Nurses for Helen Porter Rehabilitation and Skilled Nursing. In this role, you’ll deliver evidence‑based, patient‑centered care for post‑acute, rehab, and long‑term residents. Responsibilities include assessments, medication administration, wound care, documentation in the EHR, and collaboration with an interdisciplinary team. You’ll support care planning, patient and family education, and quality and safety initiatives in a collaborative, learning‑focused environment. Provide direct nursing care to rehab, post‑acute, and long‑term residents Perform comprehensive patient assessments and monitor changes in condition Administer medications and treatments per provider orders and protocols Document care accurately in the electronic health record (EHR) Collaborate with an interdisciplinary team on individualized care plans Provide wound care, IV therapy, and other skilled nursing procedures as qualified Educate patients and families on care plans, medications, and discharge needs Follow infection prevention, safety, and quality standards Supervise and support LPNs, LNAs, and other support staff as appropriate Participate in continuous improvement, staff education, and unit initiatives Patient assessment Medication administration Care planning Wound care IV therapy Electronic Health Records (EHR) documentation Infection control Patient and family education Interdisciplinary team collaboration Vital signs monitoring Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active VT RN or LPN license Current BLS certification ACLS preferred for RNs Wound care certification preferred IV therapy certification preferred for LPNs Experience in skilled nursing, rehab, or long‑term care preferred
