HIV Care & Dental Access Case Manager I

The University of Louisville School of Dentistry seeks a Medical Case Manager I to support Ryan White HIV clients across Kentucky. This full-time role involves enrolment, care coordination, and data collection to ensure competent, timely dental care through the Ryan White Oral Health Program. Responsibilities include patient enrollment, appointment attendance follow-ups, transportation logistics, and meticulous documentation in EMR and Care Ware. CQI participation is also expected.
