Clinical Pharmacy Formulary Strategist

The Tomah VA Medical Center is seeking a Formulary Management Specialist to join the Pharmacy service line. You will develop, implement, and coordinate clinical pharmacy initiatives to ensure safe, appropriate medication use for patients. Responsibilities include supporting formulary decisions, drug use evaluations, and collaboration with clinicians and leadership to achieve optimal patient outcomes and medication safety across inpatient and outpatient settings.
