Registered Nurse Care Coordinator

Hartford Healthcare seeks an RN Coordinator to support patient care and clinical operations within our Healthcare & Medical Services department. In this role, you will coordinate patient flow, provide direct nursing care, support care planning, and collaborate with a multidisciplinary team to ensure safe, high-quality outcomes. You will oversee care transitions, educate patients and families, and help optimize clinical workflows. Coordinate patient flow and clinical operations within assigned unit or service line Provide direct, evidence-based nursing care and patient education Collaborate with physicians and interdisciplinary team to develop and update care plans Oversee safe care transitions, discharges, and follow-up arrangements Monitor quality, safety, and regulatory compliance metrics Serve as a clinical resource for nursing staff and assist with orientation/precepting Communicate effectively with patients, families, and care teams to resolve issues Document care accurately in the electronic health record (EHR) Participate in performance improvement and workflow optimization initiatives Support implementation of clinical protocols, policies, and best practices Patient assessment Care coordination Clinical triage Care planning and discharge planning IV therapy and medication administration Electronic Health Records (Epic or similar) Patient and family education Interdisciplinary team collaboration Quality and safety monitoring Regulatory and compliance knowledge (Joint Commission, CMS) Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in the state of Connecticut BSN preferred BLS certification required ACLS preferred depending on unit Prior charge/resource nurse or coordination experience preferred
