Location: Marion, IN 46952 Float / Coverage Area: Wabash, Huntington, Grant, Blackford, Madison, and Delaware Counties

Contract Length: 14 Weeks

Pay Range: $48.00 to $53.00/hour*Mileage Reimbursement Rate (if applicable): $0.51 / mile*Pay offered is based on experience, expertise, credentialing, and education.   Duties: Perform comprehensive occupational therapy evaluations and reassessments in the home setting. Conduct OASIS evaluations, recertifications, discharge assessments, and routine therapy visits. Assess patients' functional status, activities of daily living (ADLs), fine motor coordination, cognitive abilities, safety needs, and adaptive equipment requirements. Develop individualized treatment plans in collaboration with physicians and interdisciplinary team members. Implement evidence-based occupational therapy interventions to maximize patient independence and functional performance. Perform home safety assessments and recommend adaptive equipment, environmental modifications, and therapeutic strategies. Observe, document, and communicate patient responses to treatment, progress toward goals, and changes in condition to physicians and nursing staff. Coordinate care with nurses, therapists, physicians, caregivers, and other healthcare professionals to ensure optimal patient outcomes. Complete accurate and timely clinical documentation in compliance with agency, CMS, and state regulations. Meet established productivity standards while maintaining high-quality patient care.

Requirements: Current Occupational Therapist (OT) License in the State of Indiana. Current CPR Certification. Minimum 1 year of recent Home Health experience. Proficiency with Home Care Home Base (HCHB)/Point Care. Strong experience completing OASIS assessments. Valid driver's license, current automobile insurance, and reliable transportation. Ability to work independently and effectively manage a field-based caseload.
