Home Health Primary Care Registered Nurse (RN)

Join Hartford Healthcare as a Primary Care Nurse providing compassionate in-home care across our Connecticut network. In this field-based RN role, you’ll deliver comprehensive assessments, coordinate individualized care plans, manage medications, and perform skilled nursing interventions for diverse adult and geriatric patients. Collaborate closely with physicians, therapists, and social workers to support safe transitions of care. Provide in-home primary nursing care to adult and geriatric patients in the community. Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and perform skilled nursing procedures, including wound care. Educate patients and families on disease management, medications, and home safety. Coordinate care and communicate with physicians, therapists, and social workers. Monitor patient progress, document in the EHR, and adjust care plans as needed. Support safe transitions of care from hospital or rehab to home. Adhere to infection control, safety, and regulatory standards in the home setting. Participate in interdisciplinary case conferences and team meetings. Engage in ongoing professional development and quality improvement initiatives.
