Clinical Navigator - Day Shift, Full-Time Rehab Referrals

PAM Health Rehabilitation Hospital of Round Rock seeks a Clinical Navigator to help grow referrals and coordinate patient assessments within our Round Rock facility. You will meet providers, build relationships, and work with the admissions team to move patients through the rehab process. The role requires a health-related degree, current licensure in a related field, and at least two years of relevant experience, preferably in LTAC or rehabilitation settings.
