Oncology Physician Assistant

Key Responsibilities: Provide direct, evidence-based primary care to patients with cancer in office and virtualsettings. Collaborate closely withoncology providers managing each patient's cancer care. Manage chronic conditionsand complex medical needs, including congestive heart failure, frailty, andbehavioral health. Coordinate referrals andcare with primary care and specialty providers in the community. Provide transitions-of-carevisits and support continuity of care. Participate ininterdisciplinary team huddles, case reviews, and care coordination activities. Escalate clinical concernsto the supervising Market Medical Director when appropriate. Collaborate with clinicaland practice leadership to improve clinical operations and patient care. Support quality initiativesand achievement of key clinical quality measures. Perform other relatedresponsibilities as needed.

Requirements: Current certification andlicensure as a Nurse Practitioner or Physician Assistant. 4+ years of direct patientcare experience in primary care, oncology, or a related setting. Strong experience in chronicdisease management and/or palliative care. Oncology orcomplex/high-acuity patient care experience preferred. Excellent clinical, communication, care coordination, and teamwork skills. Ability to work effectivelyin a growing Value-Based Care environment.
