Apply for this role →To improve health and wellness for patients with chronic conditions, the full-time remote State Licensed Health Coach will develop supportive patient relationships, monitor adherence to care plans, and communicate effectively with the cardiology care team.
Key responsibilities:
Develop and maintain patient coaching relationships that enhance care plans and quality of life
Monitor patient adherence to care plans and communicate abnormal findings to the care team
Assist new patients in the enrollment process and act as a liaison between patients and providers
Required qualifications:
High School diploma or GED
1 year of experience in remote chronic care management
Experience with cardiology patients, particularly heart failure
Registered Nurse (RN), Medical Assistant (MA), or Certified Nurse Assistant (CNA) certification
Familiarity with EMRs, specifically Athena, Nex Gen, GEMMS, and e-clinical works
