Medical Social Worker — Home Health & Care Coordination

Quality Home Health Care is seeking a Medical Social Worker to provide psychosocial support, counseling, and care management for patients at home. You will assist with resources, financial and insurance concerns, and advance care planning under the direction of a Medical Provider and in collaboration with the health care team. Requirements include a CSWE-accredited Master's in Social Work, Arizona LMSW/LCSW license, CPR certification, and at least one year in a health care setting.#
