Family Medicine/Internal Medicine Physician — Care Team Leader

Sunset Health is seeking a physician to provide professional medical services during scheduled clinical sessions and maintain a patient panel in line with Center standards. You will coordinate care and manage population health with the care team, leading the patient-centered medical home care team. On-call inpatient care when applicable, participate in CME, meetings, and quality initiatives, and serve as a preceptor to clinical students as needed.
