LaSante Health Centeris seeking anOptometristto provide comprehensive eye care services in a federally qualified health center (FQHC) settingon per diem basis. This per diem role focuses on delivering high-quality vision care while collaborating with primary care and specialty teams to support overall patient health.
Responsibilities:
- Perform comprehensive eye examinations and vision assessments
- Diagnose and manage common eye conditions and vision disorders
- Prescribe corrective lenses and contact lenses as appropriate
- Identify and refer patients with complex or surgical needs to specialists
- Educate patients on eye health, preventive care, and treatment plans
- Document all encounters accurately in the electronic health record (EHR)
Qualifications:
- Doctor of Optometry (OD) degree from an accredited program
- Current New York State optometry license (or eligibility)
- Experience in outpatient or community health settings preferred
- Strong communication and patient engagement skills
