Practice alongside Nurse Practitioners, Care Managers, Pharmacists, Social Workers, and Clinical Support Teams
Responsibilities:
Manage patients with complex health needs alongside an interdisciplinary team
Focus on prevention, chronic disease management, and lasting patient relationships
Compensation
$211,369-$317,053 base
Quarterly Incentive Plan based on outcomes, quality metrics, and documentation
Long-Term Incentive Eligible - physicians may participate in the Mosaic Health Profit Shares program

Requirements:
MD/DO from an accredited medical school
Active Arizona medical license
