Responsibilities:
Provide physical therapy services to patients in their homes, addressing physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services. Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies and procedures.
Requirements:
Minimum of 2 years of experience as a Physical Therapist, with at least 1 year of traveler experience and 1 year in home care. Valid Physical Therapist professional license in Massachusetts. Ability to travel to assigned cases within the service area. Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), and Neonatal Resuscitation Program (NRP) certifications required.
