Key Responsibilities:
- Conduct psychosocial assessments for hospice and home health patients
- Provide emotional support, counseling, and crisis intervention for patients and families
- Assist patients and caregivers with advance care planning and end-of-life support
- Connect patients with community resources, including Medicaid assistance, nutrition programs, transportation, caregiver support, utility assistance, and housing services
- Coordinate respite care services and hospice transfers as needed
- Collaborate with nurses, physicians, chaplains, and interdisciplinary team members
- Advocate for patient rights, dignity, and quality of life
- Maintain accurate documentation in compliance with hospice and home health regulations
Qualifications:
- Bachelor's or Master's degree in Social Work from an accredited program
- Active Idaho Social Work license
- Licensed Master Social Worker (LMSW) required
- Knowledge of community resources, Medicaid, and social service systems
- Strong communication, organization, and time-management skills
- Valid driver's license with the ability to travel to patient homes
Preferred Skills: Experience in hospice, home health, medical social work, or healthcare setting
