Apply for this role →To support a growing practice, the Licensed Clinical Social Worker Advocate will provide psychosocial assessments, resource navigation, and behavioral health support to members, working remotely on a 1099 contract basis with flexible scheduling options.
Key responsibilities:
Complete psychosocial and social determinants of health assessments to identify barriers affecting member wellbeing
Connect members to appropriate community resources and benefits, ensuring follow-up on resource effectiveness
Provide brief, solution-focused support and coordinate referrals to behavioral health providers when necessary
Required qualifications:
Active, unrestricted LCSW license in at least one U.S. state
Master of Social Work (MSW) from a CSWE-accredited program
2+ years of post-master's experience in medical, community, geriatric, or healthcare social work
Strong knowledge of public benefits and community resources
Self-directed with the ability to manage a member load and schedule independently
