Faith Community Nurse: Home and Community Health Advocate

BayCare's Faith Community Nursing program in Tampa, FL seeks a PRN Registered Nurse to support community health education, care coordination and transitional care visits. In this role, you will educate individuals and groups, collaborate with FCN leadership, participate in discharge planning, and visit patients at home, hospital, and nursing facilities to promote health and prevent readmissions. Weekend work is occasional and the position is PRN.
