Recent/Current HomeHealth Nursing experience: 1 year – Required Recent/CurrentHospice Nursing experience: 1 year – Required Home Care Home Base(HCHB) charting experience – Highly preferred Must have validDriver’s License and Personal Auto Insurance Must be CovidVaccinated or provide medical or religious exemption Bilingual candidates - Preferred but not required Education, Licenseand Certification Requirements: ASN or an equivalent– Required BSN or an equivalent– Highly preferred RN License for Stateof TX or Compact - Required BLS Certificationfrom AHA or American Red Cross - Required

Develop and implementnursing care plans including Start of Care and admissions as assigned. Support patientrecovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy. Empower patients andfamilies to reach healthcare goals by educating them on disease management. Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies andprocedures. Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure. Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented. Notify physicians ofsignificant changes in status. Document and completecharting for all patient visits within 24 hours. Provide bereavementsupport to patient’s families as needed. Territories to cover: San Antonio region
