Health and Wellness Director

The Health and Wellness Director leads clinical services in an assisted living and memory care community, overseeing resident health, care planning, and medication practices. This role partners with the Branch Director to maintain a safe, engaging environment, guides and coaches care staff, supports regulatory compliance, and responds to resident health changes. The director fosters strong relationships with residents, families, and providers to support each resident’s quality of life. Lead clinical operations within the assisted living community and support overall branch health services. Develop, implement, and update resident care and wellness plans in collaboration with the care team. Oversee medication management practices and monitor residents’ health status and changes in condition. Supervise, coach, and schedule care staff to ensure consistent, high-quality caregiving. Coordinate with the Branch Director on regulatory compliance, quality standards, and incident follow-up. Communicate with residents’ families and healthcare providers regarding care needs and updates. Support life enrichment and wellness programming that promotes residents’ safety and quality of life. Maintain accurate clinical documentation and participate in audits or quality improvement efforts.
