Work Environment: This role is full-time and operates onsite, primarily in patients' homes. Key Responsibilities: Conduct thorough evaluations of patients, which include reviewing medical histories and carrying out specific assessments to determine their therapeutic needs. Develop and explain individualized treatment plans to patients and their families, ensuring that their goals and safety are prioritized. Implement evidence-based treatment protocols derived from comprehensive evaluation findings, staying abreast of the latest healthcare trends and best practices. Work collaboratively with other healthcare professionals, documenting and addressing any changes in patient conditions to optimize rehabilitation outcomes. Maintain accurate and timely records of patient care activities, contributing to departmental objectives and regulatory compliance. Engage in ongoing professional development through continuing education, remaining current with industry advancements and serving as a clinical instructor when necessary. Patient Population / Client FocusThe primary focus of this role will be on a diverse patient population recovering from surgeries, injuries, or chronic conditions requiring physical rehabilitation. As a home care specialist, you will provide care to patients of varying ages and backgrounds, adapting your techniques to meet their unique needs within the comfort of their homes.

Qualifications: A Doctorate degree in Physical Therapy (DPT) is required. A valid Physical Therapist license in the State of Florida. Current certification in Basic Life Support (BLS) for Healthcare Providers issued by the American Heart Association. A strong commitment to patient-centered care, displaying excellent communication and interpersonal skills.

Preferred Experience: A minimum of two years of relevant experience in a clinical or home care setting is highly desirable. Experience with a variety of therapy techniques and modalities tailored for outpatient rehabilitation.

Compensation and Benefits: The salary for this position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
