Description:
Conduct evaluations for patients in various settings, both inpatient and outpatient, and craft tailored treatment plans that address their unique needs
Administer therapeutic interventions targeting mobility, strength, balance, and function through diverse methods
Work hand-in-hand with nurses, physicians, case managers, and other rehabilitation experts to achieve optimal patient outcomes
Guide patients and their families through therapeutic goals, discharge plans, and at-home exercise regimens
Maintain accurate documentation that reflects evaluations, treatments, and progress supporting facility and regulatory requirements
Engage in care team meetings and interdisciplinary screenings to ensure synchronized patient care
Education Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
Certifications:
State licensure as a Physical Therapist
Preferred CPR certification
Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills Prior experience is highly desirable
Ability to manage care for patients with multifaceted medical and mobility challenges
Outstanding communication and teamwork capabilities
Competence in utilizing digital documentation and clinical charting systems
