Geriatric PCP — Value-Based Care, 65+ Focus

Heartline Staffing is seeking a Primary Care Physician to provide preventive primary care for adults 65+ with a focus on Medicare Advantage, chronic disease management, and preventive care. You will see 12–15 patients per day with 20–40 minute visits in an integrated care team setting. Training is provided for Value-Based Care. Benefits begin Day 1, with compensation including base salary and bonuses; relocation assistance is available for moves >50 miles.
