Home Health OASIS Registered Nurse

Hartford Healthcare seeks an OASIS Registered Nurse – HomeCare to deliver skilled nursing care to patients in their homes. The RN completes comprehensive OASIS assessments, develops individualized care plans, and provides treatments such as wound care, medication administration, and chronic disease management. This role coordinates with physicians, therapists, and caregivers to ensure quality, patient-centered care. You’ll document in an electronic health record, educate patients and families, and support safe transitions of care. Conduct comprehensive OASIS assessments for home health patients Develop and update individualized home care plans Provide skilled nursing interventions, including wound care and medication administration Educate patients and caregivers on disease management and safety Coordinate care with physicians, therapists, and interdisciplinary team members Document assessments, visits, and outcomes in the electronic health record Monitor patient progress and adjust care plans as needed Support safe transitions of care from hospital or facility to home Adhere to infection control, regulatory, and agency standards Participate in team meetings and quality improvement initiatives OASIS assessment and documentation Home health care planning Wound care and dressing changes IV therapy and medication administration Electronic health record (EHR) documentation Care coordination and case management Patient and family education Infection control practices Clinical triage and critical thinking Time and route management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license in CT OASIS home health documentation proficiency BLS/CPR certification Valid driver’s license and reliable transportation Home health nursing experience preferred
