In-Home Nurse Practitioner (House Calls)

Essen Health Care seeks a Nurse Practitioner to provide in-home, patient-centered care focused on adults with chronic illness and mobility challenges. The NP will evaluate patients, develop treatment plans, coordinate care with a multidisciplinary team, and educate patients and families for better health outcomes. Travel to designated areas is required, with a flexible schedule and potential evenings/weekends.
