We are seeking a driven and relationship-focused Home Health Marketer / Sales Liaison to expand our presence in the Fort Worth market and surrounding communities.
Position Summary:
The Sales Liaison will be responsible for developing and maintaining referral relationships with physicians, hospitals, SNFs, ALFs, and community partners. This role is ideal for a motivated, goal-oriented professional who thrives in a fast-paced environment and is passionate about healthcare sales.
Key Responsibilities:
- Build and maintain strong referral relationships within assigned territory (Denton and surrounding areas)
- Generate qualified home health admissions (Medicare/Episodic and Non-Episodic)
- Conduct daily field visits to physicians, hospitals, case managers, and community partnersTrack and manage all activities through CRM (daily call logs required)
- Collaborate with clinical and intake teams to ensure smooth patient transitionsMeet and exceed monthly admission targets and performance goalsRepresent the agency at networking events and community outreach initiativesCompensation & BenefitsBase Salary:

Qualifications:
