PACE Driver: Transport & Care for Seniors

my Place Health, a PACE program under SCAN Group, is hiring a Driver to transport participants to appointments and meals, while ensuring safety and comfort. You will support center activities and coordinate with the interdisciplinary team to monitor participants’ well-being. We value bilingual English/Spanish abilities, a strong service mindset, and a commitment to compassionate care for seniors. This role is on-site and requires a valid driver’s license and physical capability.
