Description: Assess patients in either inpatient or outpatient facilities and formulate personalized treatment plans Implement physical therapy techniques that target mobility, strength, balance, and functionality Collaborate closely with nurses, doctors, case managers, and other rehabilitation specialists to maximize patient outcomes Guide patients and their families through therapeutic objectives, discharge arrangements, and at-home exercises Maintain accurate documentation of evaluations, treatments, and progress in adherence with facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to ensure coordinated patient care Education Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution
Certifications: State licensure as a Physical Therapist CPR certification preferred Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills Prior experience is strongly preferred Capability to manage patients with complex medical histories and mobility challenges Excellent communication and teamwork abilities Effective use of digital record-keeping and clinical documentation systems
