This role offers the opportunity to manage complex cases, contribute to continuity of care, and collaborate within a multidisciplinary team in a well-supported clinical environment. Patient Volume & Scope Approximately 6 one-hour visits or shorter visits per dayMix of new evaluations and follow-upsAdult patient population with moderate to high acuity Additional Responsibilities Participate in team meetings and quality improvement initiativesAssist with peer reviews and case consultations as neededEnsure compliance with clinical, regulatory, and documentation standardsFollow up on labs, consults, and patient communications in a timely manner
