Registered Nurse in Staten Island, NY

Job Description: This contract opportunity centers on delivering patient-focused nursing care within nursing home and assisted living environments. The role supports insurance members residing in these facilities by providing ongoing health assessments and comprehensive care, aiming to minimize unnecessary hospital visits and foster a smooth transition to home settings. The program emphasizes monthly (or as needed) health check-ins, with a strong objective of enhancing each patient's quality of life and keeping care in place whenever feasible.

Schedule: Monday through Friday, 8:00 AM 5:00 PM No weekends or holiday shifts required

Key Responsibilities: Conduct rounds across multiple skilled nursing and assisted living facilities Perform routine monthly visits at the direction of the Nurse Practitioner Monitor residents status, documenting progress towards care goals Attend meetings, represent the NP, and relay updates to the ICP within the interprofessional care team (ICT) Coordinate and collaborate on patient care activities to maximize outcomes and support transitions

Qualifications: Current RN licensure in New York Associates or Bachelors degree in Nursing (ASN or BSN) Basic Life Support (BLS) certification Demonstrated experience or strong interest in long-term or elder care environments Exceptional communication and organizational skills This position offers a competitive hourly rate of $65$80 and the chance to make a meaningful impact for patients in need of coordinated, compassionate care.
