Role Description: Conduct evaluations for patients in either inpatient or outpatient settings, curating personalized treatment plans tailored to their unique needs. Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and function. Foster teamwork with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes. Guide patients and their families by educating them on therapeutic goals, discharge processes, and home exercise routines. Maintain accurate documentation of evaluations and treatments in compliance with facility and regulatory standards. Engage in interdisciplinary team rounds and conferences to consistently deliver coordinated care. Required Education Doctorate in Physical Therapy (DPT) or equivalent degree achieved from an accredited institution.
Certifications: Must have state licensure as a Physical Therapist. CPR certification is preferred. Additional specialized certifications (such as in geriatric or neurological therapy) are beneficial but not mandatory. Skill Set Strong preference for candidates with relevant experience. Ability to assist patients experiencing complex medical and mobility challenges. Excellent interpersonal and collaboration skills. Familiarity with digital charting systems and clinical documentation processes.
