Hospice Patient Care Manager, Registered Nurse (RN)

Heartland Hospice is seeking a compassionate Patient Care Manager, RN to lead and coordinate interdisciplinary hospice services for patients with life-limiting illnesses. In this role, you will oversee clinical care planning, conduct patient assessments, manage case loads, and ensure high-quality pain and symptom management in home and facility settings. You will supervise and mentor nursing staff, collaborate closely with physicians, social workers, chaplains, and therapists, and support families through education and emotional guidance. The ideal candidate is an experienced hospice or home health RN with strong leadership, communication, and organizational skills. Oversee and coordinate hospice patient care plans across interdisciplinary team Conduct comprehensive nursing assessments and ongoing evaluations of patients Ensure effective pain and symptom management consistent with hospice standards Supervise, mentor, and schedule nursing and clinical support staff Collaborate with physicians, social workers, chaplains, and therapists Educate patients and families on disease process, medications, and care plans Maintain accurate clinical documentation and regulatory compliance Participate in case conferences and quality improvement initiatives Support families with emotional guidance and end-of-life care resources Ensure adherence to organizational policies, safety, and best practices Hospice nursing Patient assessment Pain and symptom management Care planning and case management Staff supervision and leadership Interdisciplinary team collaboration Electronic medical records (EMR) Regulatory and hospice compliance Patient and family education Crisis intervention and communication Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in state of practice BLS certification Hospice or palliative care certification (CHPN) preferred Experience in hospice, home health, or palliative care setting
