Home Health Occupational Therapist

Home Health of Western Colorado is seeking a compassionate Occupational Therapist to provide in-home therapy services across Western Colorado. This role evaluates functional abilities, develops individualized treatment plans, and delivers hands-on interventions to help patients regain independence in daily activities. The therapist will educate patients and families, recommend adaptive equipment, and coordinate care with an interdisciplinary team. Ideal candidates demonstrate strong clinical judgment, documentation skills, and a commitment to patient-centered, community-focused care in a home health setting. Provide in-home occupational therapy evaluations and treatments Develop and adjust individualized treatment and care plans Train patients and caregivers in exercises, ADLs, and safety techniques Recommend and instruct on adaptive equipment and home modifications Document assessments, progress notes, and care plans accurately and timely Collaborate with nurses, therapists, and physicians on coordinated care Educate patients and families on condition management and prevention Participate in interdisciplinary case conferences and care planning Occupational therapy evaluation and treatment Home health care planning Activities of daily living (ADL) training Functional mobility and safety training Adaptive equipment assessment and fitting Home safety and environmental assessment Electronic medical record (EMR) documentation Pain management and positioning techniques Patient and caregiver education Interdisciplinary care coordination Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Colorado Occupational Therapist (OT) license NBCOT certification preferred Valid driver’s license and reliable transportation Current BLS/CPR certification
